On April 19, 2016, Matt Polak passed away unexpectedly of natural causes at age 52. Matt was born in Chicago and moved to Logan in 1979. He was well-loved and known for his musical and artistic abilities and generous spirit. He was passionate about RC aircraft, the Chicago Bears and Blackhawks, RUSH, Les Paul guitars, and creating elaborate costumes and props for Halloween and Comic Con. He is survived by his wife, Yvonne Duncan Polak of River Heights, UT, siblings Michael Polak (Denise), Laura Hynes (Ray), and Leslie Autullo of the Chicago area, and Christopher Polak of Ogden, UT. He is also survived by parents-in-law Jim and Fiona Duncan of Kanab, UT, brother-in-law Andrew Duncan (Heather Smith) of Logan, UT, nieces Jordan, Alyssa, and Maggie Hynes and Kirsten Autullo of the Chicago area, and many cousins, aunts, uncles, and lifelong friends. He also leaves behind his three babies, Sammy, Ellie, and Cyrus ‒ his rescue cats. He was preceded in death by his father and mother, Frank and Connee Polak. Per Matt’s wishes, no formal funeral will be held. His ashes will be scattered at a future date at some of his favorite places in the mountains and in Southern Utah by friends and family.
